Medication Management Centre
Managing several medicines at the same time can be complex. The Medication Management Centre (MMC) brings relevant medication and health information together in one secure digital environment, helping patients and healthcare professionals better understand potential medication-related risks.
Developed within SafePolyMed, the MMC combines information about a person’s medicines with pharmacogenetic data and patient-reported health information. This creates a more complete picture that can support safer and more personalised medication management.
One place, multiple sources of information
The MMC connects several digital services within a single platform, including:
- Medication plans - providing an overview of current medicines
- Interaction checks - identifying potential drug-drug and drug-gene interactions
- Pharmacogenetic information - incorporating genetic results that may help explain how an individual responds to certain medicines
- Patient-reported information - capturing symptoms, health complaints and other information over time
- Medication knowledge - providing access to evidence-based information that can support medication assessment
Together, these features bring information that would otherwise be considered separately into one environment.

The MMC is designed to support collaboration between patients and healthcare professionals, not to replace professional medical judgement. The platform is intended to support active patient involvement while helping healthcare professionals identify medication-related risks.

Built for new digital services
The MMC has been developed as a flexible, interoperable platform that can connect with additional healthcare tools and plugins. Within SafePolyMed, this includes the integration of AI-based risk prediction, precision dosing and other personalised decision-support services.
Its modular approach also allows the platform to evolve as new research, evidence and digital solutions become available, providing a foundation for future research and innovation in medication safety.
The MMC is currently deployed as an experimental research platform and is not intended for routine clinical decision-making.
